Integrated Community Care Provider (ICCP) Manager

About the role

REACH Community Services Ltd is seeking an experienced ICCP Manager to lead its Integrated Community Care Provider services across four sub‑regions in Singapore. The role focuses on coordinating high‑quality senior care, building strong partnerships, and ensuring compliance with funding and governance requirements.

Key responsibilities

• Lead operations and delivery of ICCP services across assigned sub‑regions.
• Manage, coach, and evaluate the ICCP team.
• Develop and maintain partnerships with AIC, MOH, healthcare providers and community organisations.
• Oversee referrals, care coordination, KPI monitoring, reporting and documentation.
• Ensure compliance with funding, audit and governance standards.
• Create and update SOPs, support programme improvements and lead new service implementations.
• Assist with new programme start‑ups and RFP submissions.
• Represent REACH at sector meetings, committees and stakeholder engagements.

Required profile

• Recognised degree qualifying the candidate to work in Singapore in Healthcare, Social Work, Allied Health, Community Care or a related discipline.
• Minimum 3 years experience in care coordination, case management or community care programme management.
• At least 3 years of supervisory or people‑management experience.
• Experience managing AIC, MOH or NCSS‑funded programmes.
• Strong knowledge of community care operations, stakeholder engagement and programme governance.

Required skills

• Proficiency with Microsoft Office 365.
• Experience using SharePoint.
• Familiarity with Caritahub or similar care management platforms.
• Experience with BRIGHT or comparable data reporting tools.
• InterRAI Assessor certification (preferred).
