Palliative Care Staff Nurse – Home Care (East Singapore)

About the role

Metta Hospice Care is seeking a compassionate Staff Nurse to deliver home‑based palliative care in the eastern region of Singapore. You will be the clinical anchor for patients and families, providing expert symptom management and end‑of‑life support in the comfort of their homes.

Key responsibilities

• Conduct comprehensive nursing assessments for palliative patients during home visits.
• Develop, implement and review individualized care plans focused on pain relief, breathlessness, nausea, wound care and other symptoms.
• Administer medications and perform procedures such as nasogastric tube (NGT) care, catheter care and venting in line with physician orders.
• Train and support caregivers on nursing techniques, medication administration and recognising end‑of‑life signs.
• Provide psychosocial and bereavement support to families.
• Act as liaison between patients, families and the multidisciplinary team, coordinating referrals to inpatient hospices, community resources or emergency services.
• Maintain accurate clinical documentation, targeting an average of six home visits per day.
• Participate in multidisciplinary case conferences and ensure care complies with national palliative care guidelines and SNB standards.

Required profile

• Registered Nurse with a valid Singapore Nursing Board practising certificate.
• Diploma or Degree in Nursing; advanced diploma or graduate certificate in Palliative Care or Gerontology preferred.
• Minimum three years of clinical nursing experience, preferably in home care, community nursing or oncology.
• Strong clinical judgment, ability to work independently and excellent communication skills for sensitive end‑of‑life conversations.
