Social Work Associate

About the role

The Social Work Associate supports seniors in a nursing home setting by coordinating admissions, discharges, financial counselling, and psychosocial programmes. The role bridges the social work team, healthcare professionals, families, and external agencies to ensure seamless care transitions and holistic support.

Key responsibilities

• Manage admission and discharge processes, including new referrals, liaison with external agencies, and internal coordination.
• Conduct financial assessments, means testing and provide counselling on funding options such as Medifund and subsidies.
• Assist with case intake, assessment of BioPsychoSocial functioning, and ongoing case management to monitor care plans.
• Collaborate with Medical Social Workers and healthcare teams to develop Individual Care Plans and address complex cases.
• Plan, coordinate and evaluate psychosocial programmes aimed at seniors and caregivers, collecting feedback for continuous improvement.
• Maintain accurate documentation of case notes, financial assessments, and intervention records in compliance with regulatory standards.

Required profile

• Relevant training in social work, psychosocial assessment, or related fields.
• Strong interpersonal skills to interact with seniors, families, and multidisciplinary teams.
• Ability to handle confidential information with professionalism and accuracy.
