Home-based Pharmacist (NUHS)

About the role

The Home-based Pharmacist will support medication transfer processes for patients, providing counselling and ensuring safe medication use. This role combines virtual and in‑person interactions to deliver comprehensive pharmaceutical care.

Key responsibilities

• Review transfer medications and discharge prescriptions for accuracy.
• Contact patients before transfer to confirm medication supply needs.
• Answer medication‑related questions and provide counselling via in‑person or virtual channels.
• Perform medication reconciliation and verify new physician orders, flagging inappropriate doses.
• Assist with medication supply duties and follow up on preparation status.
• Undertake additional duties assigned by NUHS staff based on operational requirements.
• Work flexible weekdays and/or weekends as needed.

Required profile

• Bachelor of Science in Pharmacy from an accredited institution recognized by the Singapore Pharmacy Council.
• At least 2 years of relevant pharmacy experience.
• Fully registered with the Singapore Pharmacy Council.
• Excellent communication, interpersonal and organisational abilities.
• Ability to work independently and collaboratively within a team.
