Home Care Nurse

About the role

The Home Care Nurse provides safe, high‑quality clinical care to clients in their own homes. You will assess patients, develop and adjust care plans, and support families with education and training.

Key responsibilities

• Deliver safe nursing care and make sound clinical decisions during each home visit.
• Assess and document client conditions, updating nursing care plans as needed.
• Perform complex procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, and medication packing.
• Maintain accurate nursing documentation and report any incidents to the Nurse Lead.
• Provide caregiver training and education on home health procedures.
• Participate in quality‑improvement projects and identify service gaps.
• Support integrated home health and palliative care projects, including telesupport.

Required profile

• Diploma in Nursing.
• Minimum 2 years of experience in community nursing or home care.
• Ability to work independently with strong decision‑making skills.
• Positive, empathetic attitude with a genuine passion for client care.
• Proficiency in MS Office applications.
