About the role
You will join a multidisciplinary team focused on acute and chronic disease management to maintain health and promote patient wellness. As a senior pharmacist, you will play a pivotal role in ensuring safe, effective medication use across inpatient and ambulatory settings.
Key responsibilities
- Identify, resolve, and prevent drug‑related problems.
- Recommend and monitor patient‑specific drug therapy, ensuring safe and effective alternatives.
- Document therapeutic outcomes and oversee drug distribution processes.
- Participate actively in inpatient clinical rounds and ambulatory care clinics.
- Dispense medications and provide counseling on medication treatment.
- Communicate timely, accurate drug information to healthcare teams.
- Educate the public, patients, students, and fellow healthcare workers on safe medication use.
- Conduct clinical or translational research to advance knowledge in your specialty.
Required profile
- Eligible for registration with the Singapore Pharmacy Council.
