Associate Wellbeing Coordinator - CareLine Call Centre

About the role

SingHealth's Office of Regional Health is hiring an Associate Wellbeing Coordinator for its CareLine call centre. You will join a team of Primary Care Providers, Community Nurses, Wellbeing Coordinators and Community Partners that provides 24/7 telecare support and assistance to clients, from incoming calls to CareLine, following established protocols through virtual channels. You will receive incoming voice calls and make outgoing care calls to strengthen the care relationship with clients.

Key responsibilities

• Monitor clients' wellbeing through regular check-ins and direct them to appropriate community resources
• Coordinate health and social care administration with various stakeholders
• Maintain up-to-date documentation of care plans and encourage clients toward healthier lifestyles
• Escalate complex cases to senior coordinators when needed
• Ensure consistent service standards, run daily operational workflows and keep client data confidential
• Achieve service levels and keep abandoned call rates low through efficient call handling
• Support community outreach and engagement where required; other duties may be assigned by the supervisor

Required profile

• Fresh diploma in any discipline
• Experience in contact centres or eldercare is an added advantage
• Operational experience in health, social and community care is an added advantage
• Ability to work rotating 12-hour shifts
• Strong team player with a positive attitude and good problem-solving skills

An added advantage is being able to communicate in two or more languages and dialects.
