About the role

As a valuable member of the SCH family, you will be part of the care team providing nursing care to rehab and sub‑acute patients in a homely, unhurried environment. You will ensure continuity of care throughout hospitalisation and support patients’ transition to home and community.

Key responsibilities

Provide nursing care to rehabilitation and sub‑acute patients.
Facilitate continuity of care during hospital stay and coordinate transition to home and community settings.

Required profile

Diploma in Nursing or equivalent recognised by the Singapore Nursing Board.
Valid Registered Nurse practising certificate issued by the Singapore Nursing Board.
Valid Basic Cardiac Life Support (BCLS) certification.
Comfortable with three‑rotating shift work and deployment across three SingHealth Community Hospital sites.
Uniform required.
