Care Coordinator - Home Care

About the role

RENCI is looking for a Care Coordinator to manage referrals and provide end-to-end case management and coordination for home care clients needing Home Personal Care, Home Nursing, Home Therapy, Home Medical and Medical Escort services. The role covers client management, administrative tasks such as invoicing and reporting, and works closely with care staff, volunteers and the wider organisation.

Key responsibilities

• Manage referrals from AIC and direct referrals from community partners and the general public, and act as liaison for AIC referrals.
• Provide case management support so that admission and service delivery run smoothly.
• Keep the home care client database complete and accurate, and perform basic financial assessments and care need assessments, including home visits where required.
• Coordinate internal and external referrals of services and work with volunteers to engage clients.
• Respond to complaints and feedback from clients and next-of-kin.
• Schedule the deployment of care staff according to clients' preferred dates, times and frequencies.
• Apply for the Senior Mobility Fund, check and verify monthly invoices, submit data to ILTC and finance, and collect data for reporting to management and regulators.
• Take part in projects such as quality improvement, and carry out other duties assigned by the reporting officer.

Required profile

• Diploma in Nursing, Allied Health, Psychology, Counselling, Social Work, Sociology, Community & Social Service (Eldercare) preferred.
• At least 3 years of experience in the ILTC sector.
• Able to use local languages and dialects.
• Client-oriented, self-directed and able to work in a multi-disciplinary team with good communication skills.

Required skills

• Proficiency in Microsoft Word, Excel, PowerPoint and Outlook.
• Excellent time management, organisation and outcome focus.
