Case Coordinator Executive – Geriatric Care

About the role

The Case Coordinator Executive will lead the delivery of home‑based geriatric care across Singapore, ensuring each client receives personalized, high‑quality support. You will work closely with community caregivers, multidisciplinary teams, and external partners to create seamless care experiences.

Key responsibilities

• Conduct comprehensive client assessments and develop individualized care plans.
• Coordinate and monitor home‑based care services to guarantee timely and effective delivery.
• Provide guidance, supervision, and support to community caregivers to maintain high service standards.
• Liaise with multidisciplinary teams, healthcare institutions, and community partners for holistic care coordination.
• Maintain accurate client records, documentation, and service reports in line with regulatory requirements.
• Support quality‑improvement initiatives, audits, and operational projects to enhance care outcomes.
• Manage client enquiries, referrals, and operational matters related to home‑based care.
• Participate in community outreach programmes and organisational events.

Required profile

• Minimum Diploma in Nursing, Social Work or a related discipline.
• At least 2 years of experience in community or geriatric care settings.
• Strong interpersonal and problem‑solving abilities.
• Ability to work Monday to Friday, 9 am–6 pm.

Required skills

• Proficiency in Microsoft Office applications.
