Case Management Officer – Contract (Nephrology)

About the role

The Case Management Officer will support Project Renalssance, a program that introduces early palliative care for advanced chronic kidney disease patients. The role focuses on coordinating care, educating patients, and ensuring seamless collaboration with clinical and community partners.

Key responsibilities

• Coordinate appointments for the Kidney Supportive Care Clinic and arrange referrals to community partners such as home hospices and integrated home‑health services.
• Provide patient care, education, and counseling in line with established clinical pathways and care plans.
• Participate in multidisciplinary patient management and collaborate with physicians and other health‑care providers.
• Apply knowledge of medico‑legal risk prevention in patient care.
• Ensure proper use, maintenance, and economical consumption of medical equipment and supplies.

Required profile

• Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology or General Science.
• Registered Nurse with a minimum of three years of clinical experience.
• Renal nursing certificate or diploma is advantageous.
• Strong clinical skills, critical thinking, and ability to deliver patient‑centered care.
• Effective bilingual communication and good interpersonal abilities.
• Computer literacy with proficiency in Microsoft Office (Word, Excel, PowerPoint).
