Medical Social Worker

About the role

Ren Ci Hospital is hiring a Medical Social Worker to provide bio-psychosocial assessment and intervention for patients, residents, clients and their caregivers. The role delivers end-to-end social work services and works closely with doctors, nurses and therapists to support holistic, patient-centred care.

Key responsibilities

• Provide bio-psychosocial assessment and intervention in line with professional standards of practice
• Deliver case assessment, care planning, intervention, evaluation, counselling and emotional support
• Help address psychosocial, financial and discharge planning needs
• Develop, coordinate and run group work programmes that strengthen social support, build community networks and improve coping and resilience
• Conduct outreach to individuals and groups who could benefit from group interventions
• Guide, train and co-supervise social work undergraduates on fieldwork and newly qualified Medical Social Workers
• Take part in quality improvement projects and identify and help mitigate risks in social work practice and patient care

Required profile

• Degree in Social Work (or major in Social Work) from a recognised institution
• Accredited or eligible for Practicing Social Worker (SASW) accreditation
• 2-3 years of relevant social work experience, preferably in healthcare or community settings
• Experience in case management, psychosocial assessment, intervention and care planning
• Experience with patients, clients and caregivers with diverse and complex needs
• Experience in multidisciplinary teams and group work or community outreach is advantageous
