Pharmacist – Clinical Pharmacy Role

About the role

We are seeking a dedicated Pharmacist to join IHH Healthcare Singapore’s hospital pharmacy team. The role focuses on ensuring safe medication use, providing patient counseling, and supporting clinical teams across multiple specialties.

Key responsibilities

• Review medication orders for appropriateness and ensure accurate dispensing.
• Provide medication counseling to patients, including specialised advice on warfarin, inhaler techniques, and other therapies.
• Deliver drug information and clinical advice to doctors, nurses, and patients.
• Supervise and train pharmacy assistants and conduct quality‑assurance activities such as ward inspections and expiry checks.
• Manage controlled‑drug supply, adverse drug reaction reporting, and drug‑usage monitoring.
• Participate in medication reconciliation rounds and contribute to specialised care areas such as ICU, transplant, pain management, and diabetes care.
• Take part in projects and continuous‑improvement initiatives as assigned.

Required profile

• Degree or Master in Pharmacy.
• Registered or eligible for registration with the Singapore Pharmacy Council.
• Strong communication and interpersonal skills.
• Team‑player with a passion for patient‑centred care.
• Willingness to work rotating shifts.
