Senior Manager – Integrated Continuing Care Operations

About the role

The Senior Manager provides strategic leadership and operational oversight for Integrated Continuing Care Operations (ICCO), including Tiong Bahru Community Health Centre (TBCHC) and the EAGLEcare programme. The role drives care coordination between acute and community settings to achieve population health goals.

Key responsibilities

• Set strategic direction for ICCO teams, aligning operations with organisational objectives and population health targets.
• Lead stakeholder engagement, managing internal and external partners to deliver project outcomes.
• Assess nursing home capabilities and stakeholder readiness for care‑integration initiatives.
• Oversee programme implementation: scoping, analysis, risk management, resource utilisation, communication and project accounting.
• Ensure operational excellence at TBCHC and EAGLEcare, maintaining service standards, performance monitoring and continuous improvement.
• Collaborate with Finance to process claims, maintain audit documentation and respond to audit queries.
• Resolve patient feedback escalations and develop workflows for statistical reporting.
• Manage annual work‑plan, OPEX review and contractual agreements with external partners.

Required profile

• Bachelor’s degree with at least 6 years of relevant healthcare experience.
• Strong oral and written communication abilities.
• Analytical skills with understanding of financials, statistics and workflow engineering.
