Senior Staff Nurse - Community Mental Health Team

About the role

The Institute of Mental Health (IMH) in Singapore is recruiting a Senior Staff Nurse for its Community Mental Health Team. The nurse maintains clients in the community by delivering a comprehensive psychosocial rehabilitation programme and administering treatment for people with mental illness, ensuring continuity of holistic care through assessment, planning, implementation and evaluation of interventions for clients and their caregivers.

Key responsibilities

• Conduct home visits and perform bio-psycho-social assessments of clients and their home environment
• Plan, implement and document care plans with the multidisciplinary team, and help clients set their own rehabilitation goals
• Monitor progress, supervise medication and administer injections and depot injections as prescribed by the doctor
• Assess mental and physical status and notify the doctor of changes, and report medication errors and near misses
• Follow infection control practices, maintain resuscitation certification and store depot injections safely
• Prepare or escort clients for hospital admission and encourage adherence to follow-up and treatment
• Provide education, counselling and psychological support, and assess learning needs of clients and families
• Maintain accurate care records and train community partners

Required profile

• Diploma or Bachelor's degree in Nursing, registered with the Singapore Nursing Board
• Registered Mental Health Nurse
• Minimum 2 years' experience as a Senior Nurse in an acute ward
• Passion for community work, good interpersonal, communication, writing and IT skills, and ability to build trusting relationships with clients
• Able to work independently with good decision-making skills and to interact with clients from diverse cultural backgrounds
