Senior/Staff Nurse – Case Management

Job description
Key responsibilities
- Assess patients' care and discharge needs.
- Coordinate care plans with the multidisciplinary team.
- Facilitate discharge planning and referrals to community services.
- Educate and support patients and caregivers.
- Ensure continuity of care and smooth transition across healthcare settings.
- Maintain accurate documentation of patient care plans.
Required profile
- Possess an Advanced/Higher/Graduate Diploma, Bachelor's Degree, Post Graduate Diploma, Professional Degree, Nursing or equivalent.
- Minimum 5 years’ experience in acute care.
- Demonstrated leadership and care coordination capabilities.
- Strong stakeholder management and interdisciplinary collaboration skills.
- Experience in quality improvement projects and mentoring junior staff (advantage).
