Social Worker – Community Services

About the role

New Hope Community Services is seeking a dedicated Social Worker to support displaced and disadvantaged individuals across Singapore. You will join a collaborative team to deliver assessments, counselling, and resource linkage that empower clients to achieve stability and growth.

Key responsibilities

• Conduct intake, needs, and risk assessments for assigned clients.
• Develop and maintain case plans, documentation, and timely database updates.
• Coordinate with Family Service Centres, Social Service Offices, and other agencies for co‑management of complex cases.
• Facilitate quarterly support group sessions and community outreach activities.
• Prepare social reports, audit documentation, and submit required funder reports.
• Participate in multidisciplinary case conferences and supervision sessions.

Required profile

• Professional background in social work or related field.
• Experience engaging with vulnerable populations and navigating government and community resources.
• Strong organizational skills for case documentation and reporting.
• Ability to work collaboratively with cross‑functional teams and external partners.
