Assistant Nurse Clinician

About the role

Woodlands Hospital, an integrated acute and community hospital, is looking for a dedicated Assistant Nurse Clinician to deliver high-quality patient care and support nursing excellence in the department. The role works closely with multidisciplinary teams, patients and caregivers to provide safe, coordinated, person-centred care.

Key responsibilities

• Act as the primary nurse for patients with complex care needs, assessing, planning, implementing and evaluating individualised care plans
• Identify patients at risk of readmission and coordinate interventions and post-discharge follow-up, working with providers to support transitions to home or community care
• Lead nursing rounds and mentor, coach and develop junior nurses through bedside teaching, rounds and competency development
• Promote evidence-based practice, quality improvement and patient safety, including falls prevention, infection control, medication safety and pressure injury prevention
• Take part in audits, peer reviews and performance improvement projects, and monitor compliance with nursing policies and regulations
• Advocate for patients and caregivers, support patient and family education, and contribute to nursing policies, care guidelines and competency frameworks

Required profile

• Diploma or degree in Nursing from a recognised institution approved by the Singapore Nursing Board
• Registered Nurse with a valid Practising Certificate issued by the Singapore Nursing Board
• At least 7 to 10 years of relevant clinical nursing experience
• Demonstrated competence in managing complex patient care and leading clinical initiatives
