Associate Executive - Frailty Screening Programmes

About the role

NHG is hiring an Associate Executive on a 2-year contract to support community-based frailty screening programmes that help seniors age well, stay independent and access early interventions. The role sits within a team focused on preventive health and community care.

Key responsibilities

• Coordinate and schedule frailty screening and follow-up assessment appointments for residents.
• Contact residents by phone and SMS for appointment booking, reminders and follow-up.
• Educate and encourage seniors to take part in frailty screening and health promotion programmes.
• Manage screening records, participant databases, appointment tracking and operational reports.
• Handle enquiries from residents, community partners and healthcare stakeholders.
• Support community outreach, health talks, roadshows and onsite frailty screening operations.
• Liaise with healthcare providers, community partners and vendors to ensure smooth programme delivery and follow-up care.
• Facilitate referrals and follow-up actions for residents needing further assessment or intervention.

Required profile

• Diploma in any discipline.
• Good communication and interpersonal skills; comfortable engaging seniors and making outbound calls.
• Detail-oriented, with strong administrative and organisational skills.
• Able to work weekends and support community outreach or screening events as required.
• Prior experience in healthcare, community care, customer service, operations or event coordination is an advantage.
