Nurse Counsellor

About the role

NHG is recruiting a Nurse Counsellor to work with its group of general practitioners (GPs) in the network. The role provides structured nurse counselling, health screening, patient education on chronic diseases, mental health management and health promotion through face-to-face or virtual consultations.

Key responsibilities

• Identify patients suitable for nurse counselling so they can understand their chronic conditions and improve their health outcomes.
• Assess patients' social history, lifestyle and dietary habits to help them make effective changes.
• Work with GPs to design individualized care plans for chronic conditions.
• Advise patients on insulin injection, glucose self-monitoring, home blood pressure readings, inhaler use, diet plans and other nursing counselling.
• Conduct eye and foot screening and educate diabetic patients; perform spirometry tests for asthma or COPD.
• Assess mental well-being using questionnaires and escalate cases to GPs when needed.
• Support the admin team in tracking clinical and outcome indicators, and work with Primary Care Coordinators on patient scheduling for ancillary services.
• Support the Primary Care Network in quality improvement projects and carry out other duties assigned by the Reporting Officer.

Required profile

• Advanced Diploma in Nursing; registered nurse with the Singapore Nursing Board.
• Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management preferred.
• Minimum 4-5 years of experience, with chronic disease management experience preferred.
• Experience in health screening and comprehensive health education for patients with chronic conditions.
• Willingness to travel to the northern and central parts of Singapore and to work on Saturdays when required.

• Meticulous documentation.
• Second language or dialect is an advantage.
