About the role
We are seeking a qualified Pharmacist to join our pharmacy services team in Novena. The role focuses on ensuring medication safety, supporting clinical care, and maintaining regulatory compliance across all pharmacy operations.
Key responsibilities
- Provide operational support and leadership within pharmacy services to ensure medication safety, efficiency, and regulatory compliance.
- Deliver specialized pharmacotherapy counselling, emphasizing chronic disease management and patient quality of life.
- Coordinate daily workflow to guarantee timely, accurate, and safe medication supply.
- Review and validate prescriptions for legality, completeness, accuracy, and clinical appropriateness.
- Resolve medication‑related issues with prescribers and escalate when necessary.
- Ensure accurate dispensing, labelling, repackaging, and compounding in line with regulatory requirements.
- Perform medication reconciliation across transitions of care and document discrepancies.
- Identify, prevent, and resolve medication‑related problems through active therapy optimisation.
- Provide medication counselling and education to patients, caregivers, and healthcare professionals.
- Support discharge processes, adherence programmes, and post‑discharge issue resolution.
- Assist in inventory management, stock control, expiry monitoring, and discrepancy investigation.
- Support procurement activities, vendor coordination, purchasing, and cost management.
- Utilise clinical systems for documentation, reporting, and troubleshooting.
- Provide training, mentorship, and competency development for staff and trainees.
- Participate in quality improvement, audits, and research initiatives.
- Assist in managing medication incidents, supply issues, and service gaps.
- Ensure compliance with legal, professional, and organisational standards at all times.
Required profile
- Degree in Pharmacy.
- Registered with the Singapore Pharmacist Board and holding a valid patient‑care practising licence.
- Minimum 2 years of relevant experience, preferably in hospital, clinical, or retail pharmacy settings.
- Strong clinical knowledge of pharmacotherapy and a commitment to lifelong learning.
- Excellent organisational, analytical, and decision‑making abilities.
- Effective communication, interpersonal, problem‑solving, and conflict‑resolution skills.
- Ability to supervise, guide, and mentor junior staff and trainees.
