Senior Medical Social Worker

About the role

NHG is seeking dedicated Medical Social Workers to join its expanding team across its health institutions, including the Institute of Mental Health (IMH) and Woodlands Hospital. The main role provides direct social work interventions such as casework, counselling, groupwork, home visits and assessment interviews with patients and their families.

Key responsibilities

• Provide direct social work services including casework, counselling and care coordination
• Provide individual, group and/or family psychotherapy using evidence-based modalities
• Perform home visits to assess living environments, family dynamics and support needs
• Develop and implement intervention plans addressing emotional, social and practical challenges faced by patients
• Develop holistic treatment plans tailored to patients' psychological, behavioural and emotional needs

Required profile

• Bachelor's degree in social work
• Effective communication and interpersonal skills
• Ability to work well in a fast-paced, dynamic healthcare environment
• Commitment to compassionate, patient-centred care
• Senior position considered for applicants with relevant experience
