Senior Pharmacist (Inpatient/ED)

About the role

Woodlands Hospital, part of NHG, is looking for a Senior Pharmacist to work in its inpatient and emergency department. The pharmacist acts as a clinical advisor, educator and leader, helping to ensure safe, effective and evidence-based medication management across the continuum of care.

Key responsibilities

• Review medication orders for appropriateness, safety and therapeutic effectiveness, and resolve dosing concerns, drug interactions and adverse reactions with prescribing clinicians.
• Counsel and educate patients and caregivers on safe medication use, and provide accurate, timely dispensing.
• Give evidence-based drug information and recommendations to multidisciplinary teams and support medication safety reviews.
• Keep pharmacy operations efficient, maintain compliance with legal and regulatory requirements on medicines, and manage accurate records and cost recovery.
• Supervise, coach and mentor pharmacists, pharmacy technicians and support staff, and guide pre-registration pharmacists and students.
• Improve policies, procedures and workflows, take part in quality improvement initiatives and keep up with new clinical evidence.
• Represent pharmacy in hospital committees and projects, and take on additional work as assigned.

Required profile

• Bachelor of Science in Pharmacy or equivalent from an accredited university recognised by the Singapore Pharmacy Council.
• Registered Pharmacist with the Singapore Pharmacy Council and a valid Practising Certificate.
• Relevant hospital, institutional or clinical pharmacy experience; clinical pharmacy, medication safety or healthcare operations experience is advantageous.
• Prior supervisory or leadership experience is preferred.

Required skills

• Strong clinical knowledge of medication management, pharmaceutical care and patient counselling.
• Proficiency in Microsoft Office and relevant healthcare information systems.
• Analytical thinking, including root cause analysis and quality improvement methods.
