Senior Physiotherapist, Community Health

About the role

NHG Population Health is hiring a Physiotherapist for its Community Health team, delivering person-centred, community-based physiotherapy services that help people achieve optimal functioning and meaningful participation in daily life. Candidates with more extensive experience and senior-level competencies may be considered for a Senior Physiotherapist appointment.

Key responsibilities

• Provide clinical care for patients with complex, multifaceted needs, including biopsychosocial and environmental assessments, clinical diagnosis and individualised, evidence-based therapy plans
• Set SMART goals with patients and stakeholders and track progress using outcome measures
• Supervise and mentor junior staff and Allied Health students, and contribute to education and training initiatives
• Support or lead quality improvement and research projects; more experienced candidates may take on service planning and interdisciplinary workgroup co-leadership
• Work across various community posts and conduct home visits, requiring independent travel within the community

Required profile

• Recognised bachelor's degree or diploma in Physiotherapy
• Full registration with the Allied Health Professions Council (AHPC), Singapore, with a valid Practising Certificate
• At least 1 year of relevant physiotherapy experience
• Post-graduate certification or diploma in a relevant clinical discipline preferred
• Willingness to travel within the community and to work after office hours or on weekends when rostered
