Senior/Social Work Assistant (Contract ends Sep 2027)

About the role

NHG Polyclinics is hiring a Senior/Social Work Assistant to support patients, particularly adolescents, in their care. The position is based at Yishun Polyclinic but will be deployed to other NHG Polyclinics, and the contract runs until September 2027.

Key responsibilities

• Work with the multidisciplinary team to review and refine the Recap-lite programme work processes.
• Support programme evaluation, outcome measurement and contribute to publications and conference posters.
• Engage adolescent patients and caregivers, conduct biopsychosocial assessments and triage.
• Enrol suitable patients and provide psychoeducation, para-counselling, self-help tools and community resources.
• Escalate high-risk or complex cases to the doctor or MSW, and manage simple casework with community partners.
• Initiate clinic outreach and accompany the MSW on field visits.
• Contact patients and caregivers for appointment reminders and rescheduling.
• Document accurately in the Electronic Medical Record system and maintain programme databases, work statistics, the Eco Map and handle Recap-lite enquiries.
• Carry out other duties assigned by the MSWs or MSS-in-Charge.

Required profile

• Diligent, flexible and resourceful, with excellent interpersonal skills.
• Highly motivated and passionate about adolescent mental health.
• Team player who can work independently, patient-focused and able to thrive in a fast-paced environment.
• Willing to travel to NHG Polyclinics when required.
• Preferred: at least 4 years of relevant experience in healthcare or social services, and a certificate or diploma in community and social services or equivalent.

Required skills

• Proficiency in computer skills and Microsoft Office.
