Senior/Staff Nurse - Community Health

About the role

NHG is a community health team that brings quality healthcare directly into the homes and communities of those who need it most. As a Staff Nurse, you will work alongside healthcare professionals and community partners to support patients, residents and their caregivers, helping them navigate their healthcare journey, promote independence and wellbeing and contribute to better community health outcomes. The role offers exposure to community nursing, care coordination and population health.

Key responsibilities

• Provide person-centred care and support to patients and residents in community settings
• Conduct assessments and help develop care plans based on patients' needs
• Support patients and caregivers through education, coaching and guidance
• Work with multidisciplinary teams and community partners to coordinate care and services
• Facilitate smooth transitions of care across healthcare settings
• Monitor patients' progress and escalate concerns when required
• Take part in quality and patient safety initiatives
• Join training and development opportunities to build clinical expertise

Required profile

• Registered Nurse with a Diploma or Degree in Nursing and a valid practising licence with the Singapore Nursing Board
• Minimally one year of nursing experience
• Strong communication and interpersonal skills
• Adaptable and resourceful in a dynamic care environment
• Willingness to learn and grow professionally
