About the role
SingHealth is looking for a Social Work Assistant to support its Medical Social Workers. The role gathers patients' social, familial and financial information so that appropriate service referrals can be made.
Key responsibilities
- Support Medical Social Workers in collecting patients' social, familial and financial information to facilitate service referrals.
- Conduct home visits and interviews to ensure patients receive continuity of care after discharge.
- Determine patients' psychosocial adjustment in the community.
Required profile
- GCE 'O'/'A' level with at least 2 years' relevant experience in social services, or a diploma, preferably in Community Services or Human Services.
- Good team player with a passion for working with people.
- Excellent communication skills.
Required skills
- Proficiency in Microsoft Office applications.
