Medical Social Worker, OT, PT, Nurse Positions – Join Elderly Care Team

About the role

St John's Home for Elderly Persons is expanding its multidisciplinary team as it prepares to become a Nursing Home in late 2026. We are seeking qualified Medical Social Workers, Occupational Therapists, Physiotherapists, and Staff & Enrolled Nurses to provide person‑centred care for ambulant older adults aged 60 and above.

Key responsibilities

• Conduct functional and psychosocial assessments of residents.
• Deliver therapy sessions and support activities of daily living.
• Monitor residents' health status and coordinate care with internal and external partners.
• Participate in discharge planning, multidisciplinary case discussions, and maintain accurate documentation.
• Support programmes that promote wellbeing, independence, and social engagement.
• Uphold clinical and professional standards and contribute to quality‑improvement initiatives.

Required profile

• Relevant professional qualification and registration (e.g., degree in Social Work, recognised OT/PT qualification, valid nursing registration with Singapore Nursing Board).
• Experience with older adults or in geriatric, community, or long‑term care settings is advantageous.
• Strong interpersonal and communication skills, ability to work respectfully with diverse residents.
• Capacity to thrive in a fast‑paced care environment while maintaining confidentiality and ethical standards.
