Social Worker / Senior Social Worker – Healthcare & Community Care

About the role

We are looking for qualified Social Workers and Senior Social Workers to join a leading healthcare and community care organisation in Singapore. The role involves delivering psychosocial support, counselling and case management to seniors and individuals with health and social needs.

Key responsibilities

• Conduct psychosocial assessments and develop person‑centred care plans for clients and caregivers.
• Provide counselling, emotional support, crisis intervention and practical assistance to clients and families.
• Coordinate referrals and collaborate with hospitals, healthcare providers, government agencies and community partners.
• Assist clients and caregivers in accessing healthcare, social and financial assistance resources.
• Work with multidisciplinary teams including nurses, doctors and allied health professionals to deliver holistic care.
• Maintain accurate case documentation and prepare required reports.
• Contribute to service improvement initiatives and professional development activities.
• For senior positions, mentor and guide junior staff.

Required profile

• Post‑graduate diploma in Social Work recognised by the Singapore Association of Social Workers (SASW) and registration as a Social Worker (RSW) or eligibility for registration.
• 2–5 years of relevant experience, preferably in healthcare, eldercare, community care or case management.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate, organised and client‑centred approach.
