Senior/Staff Nurse – Home Care (NUHS@Home)

About the role

The Senior/Staff Nurse for NUHS@Home delivers acute medical care to patients in the comfort of their own homes. Working closely with physicians and multidisciplinary teams, the nurse assesses, plans, implements and monitors individualized care plans while providing education and support to patients and families.

Key responsibilities

• Assess patients on admission and daily throughout the home‑based episode of care.
• Develop, implement and evaluate personalized care plans.
• Document all nursing interventions and monitor outcomes using established charts and forms.
• Administer treatments and investigations prescribed by physicians.
• Communicate patient progress and response to treatment with physicians, nursing and allied health colleagues.
• Participate in physicians’ rounds and assist with procedures.
• Educate patients and families on disease prevention, self‑care and rehabilitation techniques.
• Ensure proper use and maintenance of medical equipment and consumables.
• Supervise, precept and guide junior nurses, nursing students and nursing assistants.
• Coordinate transitions of care within and between teams.
• Contribute to quality improvement initiatives and research activities.

Required profile

• Diploma or Degree in Nursing and a valid nursing registration.
• Experience in acute or home‑based nursing care.
• Ability to supervise and mentor junior staff.
• Strong communication and teamwork skills.
• Commitment to patient safety and continuous learning.
