Associate Consultant / Consultant – Advanced Internal Medicine (Hospital-at-Home)

About the role

NUHS@Home is an innovative hospital‑at‑home programme delivering acute, hospital‑level care in patients' residences. We are looking for an Associate Consultant/Consultant in Advanced Internal Medicine to lead clinical activities, collaborate with multidisciplinary teams, and help shape the future of home‑based healthcare.

Key responsibilities

• Provide clinical leadership and oversight to junior doctors, nurses, pharmacists and allied health professionals.
• Conduct patient assessments, develop and implement individualized care plans, and manage acute illnesses.
• Participate in daily virtual interdisciplinary rounds to review progress and update plans.
• Communicate empathetically with patients and families, ensuring understanding of care pathways.
• Utilise remote patient monitoring, video consultations, and conduct home visits with point‑of‑care investigations as needed.
• Maintain compliance with NUHS@Home policies, accurate medical records, and contribute to research and quality‑improvement initiatives.

Required profile

• Basic medical and postgraduate qualifications from an accredited university, registrable with the Singapore Medical Council.
• Successful registration with the Singapore Medical Council and accreditation by the Specialist Accreditation Board, Ministry of Health.
• Completion of advanced specialty training in Internal Medicine, Emergency Medicine or Family Medicine.
• Willingness to adopt new technologies and innovative care delivery models.
