About the role

The Home Care Staff Nurse provides professional nursing care to clients in their own residences, ensuring safe and personalized support. This role involves direct patient interaction, caregiver training, and collaboration with multidisciplinary teams.

Key responsibilities

• Deliver home‑based nursing services that require specialized clinical skills.
• Conduct on‑site training for caregivers at the client’s residence.
• Anticipate client needs and risks, offering advice to mitigate them.
• Plan, develop, deliver, and evaluate person‑centered nursing care plans in a timely manner.
• Perform initial assessments and six‑monthly reviews promptly.
• Execute care plans and interventions appropriately.
• Follow infection‑control measures according to St Luke’s Hospital policies.
• Manage patient workload through efficient planning and scheduling.
• Document visits accurately within one workday.
• Adhere to service guidelines and safety policies.
• Participate actively in home‑care multidisciplinary meetings and team huddles.

Required profile

• Registered with the Singapore Nursing Board with a valid practicing certificate.
• Minimum 4 years of recent nursing experience in medical or geriatric settings (acute or community) within the past two years.
• Experience in oncology or palliative care is advantageous.
• Strong communication and interpersonal skills with diverse stakeholders.
• Open to learning, receiving feedback, and collaborating in team environments.
• Independent, reliable, and capable of sound clinical judgment in a home‑care setting.
• Interest in supporting end‑of‑life care for homebound seniors.
• Willingness to provide after‑office‑hour tele‑consult support on a rotational basis.
• Physically able to carry work luggage and move within the community.
