Hospital Clinician (AH Hospital Clinical Programme)

About the role

Hospital Clinicians (HC) under the AH Hospital Clinical Programme (HCP) provide holistic care for patients with multiple interacting comorbidities and complex health and social needs, working alongside other healthcare team members in the wards. They combine deep knowledge in Medicine, Surgery and Emergency & Acute Care with broader domain knowledge.

Key responsibilities

• Recognise patients who are deteriorating and notify the specialist-in-charge in time, coordinating a team to assist
• Perform a targeted clinical assessment, order initial investigations and stabilise the patient
• Lead a code based on ACLS principles where applicable and arrange transfer to a higher level of care such as ICU
• Keep patients and families informed and involved in shared decision making
• Evaluate common adult presentations such as fever, chest pain, shortness of breath, abdominal pain, falls, syncope, altered mental state and acute gastrointestinal bleeding
• Initiate provisional care plans for conditions such as sepsis, stroke, heart failure, acute coronary syndrome, shock, head injury and end-of-life care
• Communicate with patients and colleagues, manage crises, obtain informed consent and support transitions of care

Required profile

• Medical staff who have completed basic specialist training and are at least PGY 5 of training for Singaporeans or PRs of Singapore
• Non-Singaporeans or non-PRs must have worked at least 5 years in a Singapore public healthcare institution
• Registered with the Singapore Medical Council
