Medical Social Worker

About the role

NUHS is looking for a Medical Social Worker to provide clinical social work intervention in counselling and support, crisis intervention and discharge planning for continuing care. The role also gives psycho-emotional and socio-economic input to the medical and multi-disciplinary healthcare team so that patients receive holistic care.

Key responsibilities

• Counselling: give emotional support to patients and family members to help them cope with illness and treatment.
• Casework: assess cases, set action plans and review progress regularly until discharge; follow up patients likely to need continued support after discharge until their social situation stabilises.
• Discharge planning: assess the psycho-emotional state, pre-admission coping, social and financial resources and available community resources, then liaise with the community to arrange post-discharge support.
• Crisis intervention: respond within the same day or by the next working day to patients in crisis such as attempted suicide or abuse, with intensive case management.
• Financial assessment and assistance for patients needing financial aid for medical treatment.
• Work with the multi-disciplinary team and liaise with community organisations for proactive case management.

Required profile

• Degree in Social Work or Graduate Diploma in Social Work.
• Prior internship or work experience in a healthcare setting is preferred.
• Commitment to teamwork in a fast-paced, innovative environment.
