Registered Nurse – Career Conversion Programme (Diploma)

About the role

This position is part of the Career Conversion Programme for Registered Nurses holding a diploma. It is a full‑time, entry‑level nursing role based at Woodlands Health, Singapore, focused on delivering safe and effective patient care.

Key responsibilities

• Perform and coordinate general nursing care for patients of lower acuity and support the RN in‑charge in the Enhanced Care Area.
• Collect clinical and psychosocial data, apply clinical reasoning, and use escalation systems when needed.
• Set measurable short‑ and long‑term goals with patients, plan evidence‑based interventions, and evaluate care effectiveness.
• Communicate critical clinical situations using SBAR and coordinate discharge planning to ensure continuity of care.
• Lead junior team members, including Enrolled Nurses and Healthcare Assistants, and adhere to hospital guidelines and safety standards.

Required profile

• Fresh or entry‑level graduate with a nursing diploma.
• Demonstrated ability to apply clinical reasoning, critical thinking, and evidence‑based practice.
• Willingness to participate in professional development and achieve continuing education requirements.
• Commitment to patient advocacy, privacy, and ethical nursing practice.
