Senior Medical Social Worker

About the role

The new Tengah General and Community Hospital, part of the National University Health System (NUHS) cluster, is being built in Tengah Evergreen Town as a comprehensive health campus offering emergency, inpatient, rehabilitative and outpatient specialist care. As a Senior Medical Social Worker you will help develop and deliver medical social work services, supporting patients and families with complex social, emotional and practical needs while collaborating with multidisciplinary stakeholders on workflows, programmes and service models.

Key responsibilities

• Contribute to planning of medical social work services, including space, design, workflow and manpower considerations
• Collaborate with clinical, allied health, operational and administrative stakeholders in planning these services
• Provide end-to-end case management support, including assessment, care planning, interventions and evaluation of care, including complex, specialised and high-risk cases
• Coordinate patient care with multidisciplinary teams
• Lead or assist in planning and implementing programmes that enhance the psychosocial well-being of the community
• Network and collaborate with community partners
• Comply with clinical and service standards and support assigned administrative duties

Required profile

• Bachelor's degree in Social Work, Graduate Diploma in Social Work or Master's in Social Work from an accredited institution
• Preferably a minimum of four years' experience as a social worker
• Registered social worker accreditation with the Social Work Accreditation and Advisory Board (SWAAB)
• Adherence to ethical principles of autonomy, beneficence, non-maleficence and justice
• Ability to multi-task and work under tight timelines
• Good analytical and organisational skills
• Excellent communication, problem-solving, presentation, negotiation and interpersonal skills
• Proficiency in additional languages or dialects is preferred; familiarity with digital tools and AI-enabled solutions is an advantage
