Senior / Staff Nurse - Specialty Care (Palliative) (Contract)

About the role

JurongHealth Campus, part of the National University Health System (NUHS), is recruiting a Senior / Staff Nurse - Specialty Care (Palliative) on a contract basis. The nurse provides patient-centred, compassionate care to nursing home residents, their families and carers, and acts as a resource for the Acute and Community Health Care Teams, Nursing Homes, hospice multidisciplinary teams and other partners. The role involves cross-site work between Ng Teng Fong Hospital (Acute and Community Hospital) and Nursing Homes, and managing a specialist palliative care caseload in the community or acute setting.

Key responsibilities

• In the acute setting: take part in assessment, care planning, implementation, evaluation and documentation of care; manage drug therapy and syringe drivers; collaborate with the multidisciplinary team on physical, psychological, emotional and spiritual support; support ward nurses and educate caregivers about end-of-life care; review care plans and coordinate end-of-life care back to the nursing home or hospice.
• In community settings: prioritise and manage a specialist palliative care caseload; review nursing care plans and attend regular reviews, case discussions and mortality reviews; follow up care plans and clarify queries for nursing home staff; help implement nursing care pathways and protocols; assist in training nursing home staff and in screening and recruiting eligible residents.

Required profile

• Diploma in Nursing or State Registered Nurse certificate.
• Relevant Advanced Nursing Diploma or equivalent preferred for the Senior Staff Nurse position.
• Relevant clinical experience.
• Good interpersonal and communication skills.
