About the role

We are seeking a dedicated Social Worker to join our team in Buangkok, Singapore. The role involves providing personalized care and support to residents in a community setting, working five days a week.

Key responsibilities

Develop individualized care plans for residents.
Execute care plans and monitor resident progress.
Design and implement programs and activities that meet residents’ care needs.
Prepare monthly review reports and interview records for approximately 50 residents.

Required profile

Degree in Social Work.
At least 1 year of relevant experience.
