Social Worker – 5 Days/Week – Buangkok

About the role

We are seeking a dedicated Social Worker to join our team in Buangkok, Singapore. The role involves providing personalized care and support to residents in a community setting, working five days a week.

Key responsibilities

• Develop individualized care plans for residents.
• Execute care plans and monitor resident progress.
• Design and implement programs and activities that meet residents’ care needs.
• Prepare monthly review reports and interview records for approximately 50 residents.

Required profile

• Degree in Social Work.
• At least 1 year of relevant experience.
