Social Worker – Nursing Home Care

About the role

We are seeking a compassionate Social Worker to join our nursing home team across Singapore. You will create and implement personalized care plans that support the wellbeing and development of our residents.

Key responsibilities

• Develop individualized care plans based on each resident’s needs.
• Implement care plans and regularly document resident progress.
• Design and organise activities and programs that address resident care requirements.
• Prepare and submit review reports and interview records every six months.
• Collaborate with healthcare professionals and other stakeholders to ensure comprehensive resident care.

Required profile

• Minimum Diploma or Degree in Social Work, Psychology, or Counselling.
• At least one year of relevant experience in the healthcare sector.
• Strong interpersonal and problem‑solving abilities.
• Proficiency in English; ability to converse in local dialects or other languages is an advantage.
