About the role
We are seeking a dedicated Social Worker to join our welfare home in Sembawang. The role involves developing individualized care plans, monitoring resident progress, and delivering programmes that meet their needs. Working hours are Monday to Thursday 8:00 am–5:45 pm and Friday 8:00 am–4:45 pm. The position offers a monthly salary ranging from SGD 3,500 to SGD 5,500. You will work five days a week, supporting approximately 50 residents.
Key responsibilities
- Develop individualized care plans for residents and execute them while monitoring progress.
- Design and implement programmes and activities that address residents’ care needs.
- Prepare six monthly review reports and maintain interview records for about 50 residents.
- Collaborate with multidisciplinary teams to ensure holistic support for each resident.
Required profile
- Degree in Social Work.
- Preferably experience in the Social Service sector or a related field.
