Associate Consultant/Consultant, Advanced Internal Medicine (NUHS@Home)

About the role

NUHS@Home is a hospital-at-home programme that provides hospital-level acute care to patients in their own homes, using remote patient monitoring, video consultation, home visits and point-of-care investigations. It is part of the Division of Advanced Internal Medicine, Department of Medicine, National University Hospital. The role involves leading and overseeing the NUHS@Home team while working with medical and allied health professionals to deliver high-quality care.

Key responsibilities

• Provide clinical leadership and oversight to the NUHS@Home team, including junior doctors, nurses, pharmacists and allied health professionals.
• Conduct patient assessments, develop and implement care plans, and manage acute illnesses.
• Take part in daily virtual team rounds to review patient progress and update care plans.
• Communicate clearly and empathetically with patients and their families about their care plan.
• Use remote patient monitoring and video consultation to support patient care and safety.
• Carry out home visits and point-of-care investigations as required.
• Follow NUHS@Home policies and procedures and keep accurate medical records.
• Take part in research and quality improvement initiatives.

Required profile

• Basic medical and postgraduate qualification from an accredited university or school, registrable with the Singapore Medical Council.
• Registered with the Singapore Medical Council and accredited by the Specialist Accreditation Board, Ministry of Health.
• Advanced specialty training in Internal Medicine, Emergency Medicine or Family Medicine.
• Willingness to embrace new technologies and innovative ways of delivering care.
