Care Navigation Associate / Senior Associate

About the role

NTUC Health, a provider of health and eldercare services in Singapore, is looking for an Associate or Senior Associate for its Care Navigation team. You will help customers understand their circumstances and recommend suitable services, with training provided.

Key responsibilities

• Demonstrate a strong understanding of eldercare services that meet the needs of seniors.
• Answer customer calls and email inquiries.
• Assess customers' needs, recommend suitable services and facilitate their enrolment.
• Carry out needs assessments, including observations and interviews with seniors and their families.
• Work closely with service providers to ensure timely enrolment and ongoing support.
• Take part in meetings, take minutes and track follow-up actions.
• Prepare reports for regulators and other stakeholders.
• Represent NTUC Health at occasional roadshows promoting eldercare services.

Required profile

• At least 2-3 years of relevant experience.
• Diploma or degree in any discipline.
• Prior healthcare or community care experience is an advantage.
• Patient and enjoys helping seniors and their families.
• Meticulous and detail-oriented.

Required skills

• Tech-savvy, able to learn and use various IT systems.
• Strong communication skills.
