Community Health Coach

About the role

NUHS Population Health & Community Care (PHCC) is looking for a Community Health Coach to support and empower residents in western Singapore through personalised health coaching. The coach helps people adopt healthier lifestyles, manage chronic conditions and improve quality of life, including following up on health plans set by primary care providers and community nurses and taking part in health programmes run by community partners such as Active Ageing Centres.

Key responsibilities

• Perform health and social screenings to identify residents' health needs
• Develop a personalised health plan and coach residents toward their health and wellness goals
• Help residents overcome barriers to healthy behaviours and lifestyle changes
• Document and maintain accurate records of residents' progress
• Escalate care gaps and issues to community nurses, and escalate complex needs following the escalation matrix
• Collaborate with community nurses and Community Health Managers to support residents
• Assist residents in navigating social and health systems
• Provide basic patient and family education on wellness and disease prevention
• Perform preventive health screenings in the Western region and take part in health talks and other health promotion events

Required profile

• Diploma or advanced diploma, or relevant certification, in the healthcare industry
• Healthcare experience is an advantage
• Knowledge of community services is an advantage
• Able to work both in a team and independently
• Strong communication and social engagement skills, especially with older adults
• Strong organisational, coordination and interpersonal skills
• Ability to converse in local dialects is preferred
