Community Nurse, Day Care (Serangoon)

About the role

NTUC Health is recruiting a Community Nurse for its Serangoon senior day care centre. You will provide a high standard of care through care management, assessment, planning, implementation and evaluation of nursing care to meet the needs of clients, including educating family members, while delivering cost-effective care in line with established policies, standards of nursing care and practices.

Key responsibilities

• Assess the needs of clients and deliver quality geriatric and dementia nursing care
• Perform pre-assessment of new clients referred by AIC and ensure enrolment complies with company SOPs
• Submit the required documentation to the relevant government body
• Coordinate with the healthcare team to assess, plan, implement and evaluate each client's condition
• Monitor and review clients' progress and implement care plans
• Provide health promotion and counselling to clients and families
• Perform nursing care such as medication administration and wound dressing, and other technical skills as required
• Oversee the performance of the nursing team and care staff to ensure the desired level of competency
• Monitor clinical and service quality indicators and contribute to continuous improvement
• Perform CPR, AED and first aid and play an active role in emergencies
• Support health promotion activities at the centre
• Secondary duties include clinical service projects, infection control campaigns, training and development of care staff, and additional tasks assigned by the Senior Nurse Manager

Required profile

• Registered Nurse with the Singapore Nursing Board
• Degree or Diploma in Nursing
• An Advanced or Specialist Diploma in Geriatric or Community Nursing is an added advantage
• Minimum 3 years of relevant work experience is preferred
• Able to work 4 weekdays and 1 Sunday
• Strong sense of integrity, pleasant and cheerful disposition, good communication and interpersonal skills, able to work independently and with a team
