About the role

The Home Care Staff Nurse provides professional nursing care to clients in their own homes, ensuring safe, person‑centred services. This role involves direct patient care, caregiver training, and collaboration with multidisciplinary teams to support seniors, including those requiring end‑of‑life care.

Key responsibilities

• Deliver home‑based nursing services that require specialized clinical skills.
• Conduct on‑site training for caregivers at the client’s residence.
• Assess client needs, anticipate risks, and advise on mitigation strategies.
• Develop, implement, and evaluate individualized nursing care plans.
• Perform initial assessments and six‑monthly reviews promptly.
• Execute care interventions and maintain infection‑control standards per St Luke’s Hospital policies.
• Manage patient workload through efficient planning and timely scheduling.
• Document visits accurately within one workday.
• Participate in home‑care multidisciplinary meetings and team huddles.

Required profile

• Registered with the Singapore Nursing Board with a valid practising certificate.
• Minimum 4 years of recent nursing experience in medical or geriatric settings (acute or community) within the past 2 years.
• Experience in oncology or palliative care is an advantage.
• Strong communication, interpersonal, and teamwork skills.
• Ability to make sound clinical judgments in a home‑care environment.
• Willingness to support end‑of‑life care and after‑hours tele‑consultations on a rotational basis.
• Good health to carry work luggage and move within the community.
