Senior / Staff Nurse (SOC - Diabetes Clinic)

About the role

JurongHealth Campus, part of the National University Health System (NUHS), is recruiting a Senior / Staff Nurse (SOC) for its Diabetes Clinic. The nurse provides high-quality patient care, working closely with the endocrinology team to support people living with diabetes.

Key responsibilities

• Assess patients' social history, lifestyle and dietary habits to give tailored counselling and help them understand and manage their diabetes.
• Work closely with the Endocrinologist to develop personalised care plans for effective diabetes management.
• Deliver patient education on insulin injection techniques, glucose monitoring (glucometer or continuous glucose monitoring), review of glucose profiles and other diabetes-related counselling, in both inpatient and outpatient settings.
• Support the clinic's quality improvement initiatives to achieve better clinical and process outcomes.
• Carry out any other duties assigned by the Reporting Officer.

Required profile

• Diploma in Nursing or State Registered Nurse Certificate.
• Specialist Diploma in Diabetes Education or Certified Diabetes Educator certification preferred.
• Minimum 2 years of relevant experience, with a focus on chronic disease management.
• Demonstrated experience giving comprehensive health education to patients with diabetes.
• Proactive attitude, strong team spirit and professional maturity.

Required skills

• Excellent clinical judgment, critical thinking and problem-solving skills to deliver care in line with established protocols.
• Experience in diabetes education and chronic disease management.
