Social Work Associate

About the role

The Social Work Associate supports seniors and their families through comprehensive case management, admission and discharge coordination, and psychosocial programme delivery. This role blends direct client interaction with administrative duties to ensure smooth transitions and optimal care outcomes.

Key responsibilities

• Manage admission and discharge processes for seniors, including referral handling, liaison with external agencies, and coordination with internal teams.
• Conduct financial assessments and means testing, providing counselling on funding options such as Medifund and other subsidies.
• Assist in case intake, gather BioPsychoSocial information, monitor care plans, and follow up on interventions.
• Collaborate with Medical Social Workers and healthcare professionals to address complex cases, including mental health concerns and family conflicts.
• Plan, coordinate, and evaluate psychosocial programmes that promote wellbeing and community integration for seniors and caregivers.
• Maintain accurate documentation of case notes, financial assessments, and intervention records in compliance with organisational standards.

Required profile

• Training in psychosocial assessment and intervention.
• Experience supporting seniors and families through admission, discharge, and financial counselling.
• Ability to work collaboratively with multidisciplinary teams and external agencies.
• Strong organisational skills and attention to detail for documentation and programme coordination.
