Temp Assistant Care Coordinator (Healthcare)

About the role

A temporary assistant care coordinator is needed in a healthcare setting in Jurong East, Singapore, on an 8-month contract. The role supports patient enrolment, follow-up and coordination with care teams, working Monday to Thursday 8.30am to 6pm and Friday 8.30am to 5.30pm.

Key responsibilities

• Contact patients about enrolment and follow up with those who have not responded.
• Monitor chatbot responses and escalate patient concerns to the clinical team according to protocol.
• Coordinate appointments, referrals and handovers between patients and care teams.
• Maintain patient records, track service data and prepare reports.

Required profile

• Minimum diploma or experience in healthcare administration, care coordination, nursing support or a related field.
• Comfortable speaking with patients and using multiple IT systems, spreadsheets and dashboards.
• Organised, detail-oriented and able to handle patient information confidentially.
• Experience in patient-facing healthcare, telehealth or chronic disease management is an advantage.
