Transplant Coordinator, Kidney Transplant Programme

About the role

The Transplant Coordinator supports the running of the Hospital's Kidney Transplant Programme, performing both clinical and administrative work. This includes coordinating patient referrals and pre-transplant evaluations, and liaising with legal authorities and Ministry of Health officials on policies related to organ donation and transplant, ensuring compliance with the law.

Key responsibilities

• Attend to and evaluate referrals from renal physicians for live and deceased donor kidney transplants from NUH, other restructured hospitals and private hospitals in Singapore (overseas referrals only for live donor transplants)
• Assist potential donors and recipients through the whole transplant process, including counselling and transplant workup, and counsel families considering live donor transplants
• Counsel patients and families with no overt contraindications who are considering deceased donor transplants
• Maintain close contact with stakeholders and MOH officials on organ donation and transplantation
• Keep the waiting list suitable by scheduling laboratory investigations, tracking results, scheduling appointments and arranging clinic reviews
• Evaluate and prepare a patient for a deceased donor transplant once selected
• Attend ward rounds and clinic sessions, help schedule investigations in line with the transplant protocol, and provide discharge teaching and follow-up care for complex post-transplant patients

Required profile

• Registered nurse with at least 5 years' experience preferred (ICU nursing or OT post-basic qualification an advantage), or a Bachelor's degree in Psychology, Social Work or Life Sciences
• At least 2 years' relevant experience, preferably in healthcare
• Mature and pleasant personality, with drive and initiative
• Able to work independently and under pressure, willing to be on-call
